Look up page is blank

There are a few possible causes for which the Order look up page might be blank:

• The Order Lookup app is not installed
• There's Javascript code on the page that may be preventing Order Lookup from loading
• There's CSS code on the page that may be preventing Order Lookup from loading
• Your store is using cloudflare.com as a proxy (support for cloudflare.com users)
